is a person who accompanies a People with disability
(PWD) to provide services related to the disability

(service units) and Haven of Hope Hospital (Pulmonary
Rehabilitation Centre and other Community Services)
*Participant must be a registered delegate.
For this Local Health & Social Service Agency Visit,
the fee is HK$100 per person which will be used for
arranging transportation.
Assemble Place and Time:
The Hong Kong Polytechnic University (Jockey
Club Auditorium) at 8:15 a.m.
*Coach departs at 8:25 a.m. and returns to the Hong
Kong Polytechnic University at 1:00 p.m.

Upon receipt of the completed Registration Form and
full payment, the Secretariat will issue a letter of
confirmation by email. Please bring along the letter
with you when you check-in at the Registration Counter.
Payment of registration fees must be made in Hong Kong
