Registration fee includes: Admission to all sessions except optional workshops and field visit, conference materials, coffee/tea breaks and Opening Ceremony. Lunch is NOT provided.

# 1 - Local members from the organizations of the Organizers, Co-organizers and Supporting Parties.
# 2 - Student must be a full-time student of recognized nursing/medical school, social work and etc. Proof of status issued by their institutes is required. Limited seats are allocated to students. Registration is on first-come-first-served basis.
# 3 - People with disability (PWD) may be required to produce doctor's certificate for proof. An attendant is a person who accompanies a People with disability (PWD) to provide services related to the disability in attending the conference. No conference materials will be provided to the attendant.

Field Visit on 11 January
To the Hong Kong Society for Rehabilitation (service units) and Haven of Hope Hospital (Pulmonary Rehabilitation Centre and other Community Services)
*Participant must be a registered delegate.
For this Local Health & Social Service Agency Visit, the fee is HK$100 per person which will be used for arranging transportation.

Assemble Place and Time:
The Hong Kong Polytechnic University (Jockey Club Auditorium) at 8:15 a.m.
*Coach departs at 8:25 a.m. and returns to the Hong Kong Polytechnic University at 1:00 p.m.

Registration Procedure

Participants may register for the Conference by the following methods:

B. Complete the registration form and return it to the Secretariat by fax or by mail with appropriate payment.

To enjoy the early bird rate, registration with payment must be received no later than 30 November 2009.

Upon receipt of the completed Registration Form and full payment, the Secretariat will issue a letter of confirmation by email. Please bring along the letter with you when you check-in at the Registration Counter.

Payment

Payment of registration fees must be made in Hong Kong Dollars by the following methods:

1. Cheque payable to: “ Hong Kong Society for Rehabilitation ”
(Payment in form of cheque must be sent together with a completed registration form.)
2. Credit Card: Visa / Master Card

Cancellation Policy

Refund of cancellations of registration will be made subject to the following deadlines and administrative charges:

On/before 30 November 2009 50% of the registration fee
After 30 November 2009 No refund

All cancellations must be made in writing to the Secretariat at reg@Chronic-Care2010.org and the refund will be made after the Conference. Please note that substitution is permitted on the basis that written notice is received before 30 November 2009.

Notes:

1. Each registrant should complete a separate registration form. Photocopy of the registration form is acceptable.
2. Registration form without payment will NOT be processed. Please do NOT send cash.
3. Secretariat will send a letter of confirmation by email upon receipt of your registration form and full payment. Kindly check all the listed items. Any changes or alterations must be made in writing to the Secretariat.
4. The programme is subject to change without prior notice. In the event of cancellation of the Conference, the only liability of the Organizers is to refund all the registration fees paid.
5. The Workshops may be cancelled due to insufficient enrolment. Refund will be made after the Conference.
6. The prevailing exchange rate is approximately US$1 to HK$7.80
